Referrals are accepted from general practitioners (GPs), hospital and community clinicians, residential aged care staff, family members, carers and self-referrals. We do require a written referral from a GP/Medical Practitioner. Please click here to download our GP Referral Form. Internal referral via web delacey. An ‘intake officer’ (psychogeriatric clinician) will respond to and triage all referrals to the psychogeriatric service. Who can refer to the Dementia Team? We accept referrals from anyone with an interest in the person We don't require a GP/Medical Practitioner referral, however we work with the patient, family, GP and referrer to ensure that effective communication is maintained. Common referrals the Psychogeriatric Team receive include: Mental health assessment Cognitive assessment/dementia diagnosis Behaviour management support Medication review Capacity assessment e.g. for purpose of guardianship (note: not for medicolegal) Psychological therapy: individual or group Carer support Psychoeducation Common referrals the Dementia Team receive include: Initial assessment process of obtaining a formal diagnosis of dementia Management and advice for patients and families after formal diagnosis Management of Behavioural and Psychological Symptoms of Dementia Complex case management Carer support (individual or group) Psychoeducation
