External Referral

Hospital in the Clinic (HICL) is an outpatient service which provides both Chronic and Acute wound care and infusion therapies to provide ongoing care for your patient’s that are unable to be managed in a General Practice.  Hospital in the Clinic is part of an Emergency Department prevention strategy as well as helping to facilitate early discharge.

Services offered:

  • Wound Care
  • IV Antibiotics (via Hospital in the Home (HITH) referral)
  • Infusions
    • All infusions require referral to a consultant with admitting rights for St Vincent’s hospital
    • If you would like to refer a patient for an IV medication, please address your referral to the Nursing Unit Manager, Ambulatory Care
    • All referral are triaged and discussed with the Medical Director.

Who can use this service?

  • people who are aged 16 years or older
  • Ambulant (if non-ambulant please discuss with Nursing Unit Manager or in-charge before referring)
  • Hospital in the Home (HITH) patients

Who is unable to use this service?

  • Patients outside St Vincent’s Hospital catchment

All referrals require:

  • Demographics (3 to identify the patient)
  • Medical History
  • Reason for referral

Other specific requirements on referral

  • Wound care
    • Wound care chart
    • Relevant diagnostic results which may include
      • Swab results
      • APTI’s/ duplex scan results
