Hospital in the Clinic is part of an Emergency Department prevention strategy as well as helping to facilitate early discharge. Services offered: Ferinject ®* Wound Care Ferrous Iron IV Antibiotics (via Hospital in the Home (HITH) referral) Blood products Biologic(s) including infliximab Zometa Non-cancer Chemotherapy (if not linked to Heart Lung or IBAC) Who can use this service? people who are aged 16 years or older Ambulant (if non-ambulant please discuss with Nursing Unit Manager or in-charge before referring) Discharged patients from Wards/Emergency Department and other services Catchment (if they are outside St Vincent’s catchment, please consider if there is a local service that would be more appropriate) Hospital in the Home (HITH) patients Who is unable to use this service? Current Inpatients admitted to the wards, Sacred Heart or ED Referral requirements Web DeLacy (preferable) or via FlexICare Referral Form How do I refer? Call the O'Brien Referral Centre (ORC): 83821450 OR Fax the referral to: 02 8382 1997 Or Email the referral to orc@stvincents.com.au All referrals require: Demographics (3 to identify the patient) Medical History Reason for referral Other specific requirements on referral Wound care Wound care chart Relevant diagnostic results which may include Swab results APTI’s/ duplex scan results Freinject Referral Medication chart (NSW Health Regular Medication Chart or NSW Health Long Stay Medication Chart Recent HB Recent Iron studies Outside script Copy to be faxed on referral patient will need to purchase Ferinject at a community pharmacy and bring it to their appointment) Please call HICL on 83823163 to make an appointment
