Referral to a Diabetes Specialist

The SVHS Endocrinology and Diabetes Department adopted the NSW State-wide Referral criteria for selected conditions. Any referrals require the minimum clinical information to enable safe, timely and effective triage. Clinical Prioritisation Criteria are applied to determine how quickly the patient should be seen

Referrals accepted from: General Practitioners or Specialists

Eligibility:  Patients with diabetes aged 16 years and over

STEP 1: Address referral to one of the following Specialists:

  • Prof Roger Chen
  • Prof Jerry Greenfield
  • Dr Joel Lasschuit
  • Dr Nele Lenders
  • Prof Kathy Samaras
  • A/Prof Alex Viardot
  • Dr Jennifer Snaith

STEP 2: Please include type of diabetes, past medical history, current medication, recent HbA1c and any other relevant information.

STEP 3: Email referral to: svhs.diabetesfax@svha.org.au

      Referrals are triaged for clinical urgency by a clinician.

STEP 4: The Diabetes Centre staff will contact the patient to arrange an appointment.

 

 For urgent referrals please call and discuss with the Diabetes Registrar or Consultant on call:

  • During office hours (8.00am- 4.30pm, Mon-Fri, excluding public holidays) please call the Diabetes Centre directly on (02) 8382 2622.
  • Out of office hours please call the hospital switchboard on (02) 8382 1111, and ask to speak to the “Diabetes Doctor on call”.
