Nursing You are here:HomeResearch & EducationUNDA Student Compliance Verification Prior to attending clinical placement in any NSW Health facility students are required to provide evidence of compliance with the NSW Health Student Placement policy which includes providing specific documentation related to criminal record check and vaccination. Verification documentation required Documents required are listed below and in the order that they are to be submitted. UNDA Student Photo Identification Card Australian National Police Record Check OR Clinical Placement Authority Card Issued by NSW Health (all students Australian residents and International Students) For International Student Criminal Record Check from country of origin OR signed Statutory Declaration Completed and signed Undertaking/Declaration Form (Appendix 6) Completed and signed NSW Health Code of Conduct Agreement Completed and signed TB Assessment Tool (Appendix 7) Evidence of Vaccination for the following diseases as detailed in the table below. Vaccination Evidence of the above vaccinations can include: Vaccination Record Card (filled in by GP or practice nurse with their name, signature & practice stamp) Childhood vaccination card Medicare Immunisation History Statement Statement of evidence from GP Serology reports See this example of how documentation should be presented for review All NSW Health Forms mentioned below can be found via the links in the above list or at: https://www.heti.nsw.gov.au/Placements-Scholarships-Grants/clinical-placements/student-compliance Presentation of documentation Documents must be in colour and submitted in one PDF file and in correct order. All documentation must match the student card and ClinConnect unless a marriage certificate or change of name certificate has been provided. All correspondence must be sent from your university email account. If follow up documentation is required, all previous documentation previously provided and new additional evidence is required to be sent in one PDF file. (see example above) Please label the file: Student ID number, SURNAME, First name. Verification Documentation e.g: 1111111, CITIZEN, John. Verification Documentation When forwarding follow up documentation, email subject headings must include: Student ID number, SURNAME, First name. e.g. 1111111, CITIZEN, John Submission of documentation New Students Upload your Verification Requirements/Pack to SONIA under the ‘Checks’ tab. In the dark blue ‘ClinConnect (NSW Health) Verification Requirements/Pack’ section, ‘Submit Verification Requirements/Pack Here (PP1 & EN-BN ONLY)’ grey tile. Previous Students Upload any outstanding requirements to SONIA under the ‘Checks’ tab. In the light blue ‘ClinConnect Updates’ section, ‘Submit Updated Documents Here (PP2–PP6)’ grey tile. For any tiles that are red under the ‘ClinConnect Updates’ section, upload the supporting evidence to the above-mentioned submission tile, plus all relevant previously submitted documentation/evidence (previous Hep B vaccination/s for example). Influenza Upload to SONIA under the ‘Checks’ tab. In the light blue ‘ClinConnect Updates’ section, upload to the ‘Submit Influenza Proof Here’ tile’s ‘Documents’ upload section. Feedback If you are compliant for clinical placement, with all tiles green and no red tiles, no further action is required for NSW Ministry of Health Requirements (Part A). Students may receive feedback on their submissions requesting additional evidence. Check your submission regularly for updated tile colour or feedback
