Before you can attend clinical placement in any NSW Health facility you need to provide evidence of
compliance with the NSW Health Student Placement policy which includes providing specific documentation.
Verification documentation required
Documentation must include the following and in the order listed below.

Serology Evidence |
| Diphtheria, Tetanus, Pertussis |
One adult dose of diphtheria / tetanus / pertussis vaccine (dTpa) within
last 10 years (Not ADT).

Serology will not be accepted |
| Hepatitis B |
History of age-appropriate course of Hepatitis B vaccine,
and
evidence of anti-HBs ≥ 10mlU/ml.
“Accelerated” course not accepted.
|
Completed hep B Statutory Declaration (Appendix 9) confirming verbal history
and evidence of anti-HBs ≥ 10mlU/ml.

| Measles, Mumps, Rubella (MMR) |
2 doses of MMR vaccine at least one month apart, OR
|
Positive IgG for Measles, Mumps and Rubella
|
| Varicella |
2 doses at least one month apart
(one dose acceptable if vaccinated before 14 years of age), OR
|
Positive IgG for varicella |
| Influenza |
Seasonal Southern Hemisphere dose required
from March to September each year
|
|
Vaccination evidence of the above vaccinations can include:

Vaccination Record Card
(completed by GP or practice nurse only)
- Childhood vaccination card
- Medicare Immunisation History Statement
- Statement of evidence from GP
- Serology reports

Presentation of documentation
- Documents must be submitted in colour as one PDF file, in correct order
- All documentation must match the student card and ClinConnect records
- All correspondence must be sent from your university email account
- Follow-up documentation must include all previous and new evidence in one PDF
