It is essential that all referrals include information that will allow the triage process to occur in a timely manner. All referrals should include the following information: Patient’s full name Patient’s full address Patient’s telephone contact number’s – (home, mobile) Patient’s date of birth Patient’s Medicare number Interpreter required? If yes, language spoken Name of specialist to whom the patient is being referred Reason for referral Medical History Relevant diagnostic results and current medications Once the referral has been triaged the patient will be notified by mail or phone of their upcoming appointment.
