Chronic & Complex Care (CCC)

The Chronic and Complex Care (CCC) service supports people living with chronic health conditions to maintain independence and wellbeing at home. We deliver person-centred care throughout the St Vincent's Hospital community catchment area.

We work with you, your family, and healthcare providers so you have access to the right healthcare services and community resources to support your health. We give you education and strategies so you can manage your health condition at home.
