What is the Hospital in the Home (HITH) Service?

St Vincent's Hospital in the Home (HITH) is a continuation of your hospital care but in the comfort of your own home, workplace or in our onsite clinic. HITH is a free, short-term care service for conditions requiring no more than 7 days of hospital care.  A range of clinical conditions can be effectively and safely managed without you needing to be in hospital. 

While choosing HITH is entirely voluntary, it's important to know that you are still treated as an admitted patient, with the same standard of care that you'd get if you were staying onsite with us.

 

How does the service work? 

The HITH team include doctors, nurses and allied health professionals. When you are admitted to HITH, all home/clinic appointments will be arranged for you. 

We will review you every day at your home or within our clinic, or in some cases via a virtual platform (e.g., phone or computer).

During your HITH admission, one appointment a week will be at the St Vincent’s onsite clinic for a medical review with the potential for tests, scans and/or review by a specialist doctor from your treating team. We will send a discharge letter to your GP and/or specialist about your treatment with us upon discharge. 

Why would I choose HITH instead of staying in hospital?

There are many benefits to being treated by the St Vincent's HITH team. You will receive the same quality of care that you would in hospital, but in the comfort of your own home. You will be able to eat your own food when it suits you,  sleep in your own bed with no disruptions, and  be surrounded by loved ones. 

How do I know if I'm eligible?

A range of clinical conditions can be effectively and safely managed in your own home. Ask your treating doctor if you are eligible - if you are not, ask them to ensure they let you know once your condition stablises and you can be transitioned to HITH for the duration of your treatment.

Your HITH Coordinator is available 8am - 8pm. For any patient concerns or to reschedule appointments, call 0437 654 355.

After Hours (non-emergency) 8pm - 8am. If you have concerns about your condition, call the HITH on-call Doctor through Hospital Switch on (02) 8382 1111.

In an emergency, please call an ambulance 000 (triple zero) or visit your nearest Emergency Department.

Who can submit the referral?

Any clinician directly involved in the care of the patient

We accept referrals from General Practice, Residential Aged Care Facilities, Private and Public Hospitals. We do not accept self-referrals.
