Chronic & Complex Care Service

The St Vincent's Chronic and Complex Care Service is an outpatient service which provides time-limited goal oriented care for people with chronic and complex health and social care needs. 

These patients often require comprehensive assessment, care planning and review to prevent unplanned hospital admissions.

Who can use this service?

  • People who are aged 16 years or older
  • Able to self-manage or have carer
  • Diagnosed chronic and complex health condition/s
  • Within St Vincent’s Hospital Sydney Catchment Area

Patients outside St Vincent’s Hospital catchment are not eligible.

Referral Form

Referrals should include:
  • Community and Ambulatory Service Referral Form
  • Demographics (3 to identify the patient)
  • Medical History
  • Reason for referral
Other specific requirements on referral:
  • Wound care chart
  • Relevant diagnostic results which may include
  • Swab results
  • APTI’s/ duplex scan results
