Our care is personal, and focused on what matters most to you. We can support you with: A full health check-up We will listen to your concerns, assess your health, and work with you to make a care plan that fits your life. Support to manage your condition We’ll teach you how to monitor your health, notice early warning signs, and take steps to stay well. Coordinated care We make sure your GP and other health services are involved in your care. What to expect from the program Most people take part in our program for about three months. During this time, we provide: Specialist care Support from nurses and allied health experts to manage your health and any urgent issues. Education and tools Easy-to-understand information and tips to help you manage your condition every day. Health coaching Help to set your own goals, make lifestyle changes, and stay motivated. Ongoing coordination Help connecting with other health services and local supports. Flexible care options We offer flexible options to make your care easier: Home visits – we come to you Telehealth – care by phone or video call Outpatient clinics – appointments at the hospital
